Transitions & preventionAWV

Every eligible Medicare patient, scheduled and prepared.

The AWV is a yearly prevention-planning visit — risk assessment, screening schedule and goals. Most practices leave many eligible patients unscheduled. We find who is due, book them, collect the health-risk assessment ahead of time and prepare the chart so the visit runs on time.

How we run it

What the Carevory team does every month.

Your clinicians keep every clinical decision. We run the operations around them.

01

Build the due list

We identify patients eligible for an initial or subsequent AWV.

02

Reach out

Calls, texts and letters invite patients through your approved channels.

03

Collect the HRA

Health-risk questions are gathered before the appointment where possible.

04

Prep the chart

Open care gaps, screenings and immunizations are summarized for the clinician.

05

Follow through

Ordered screenings and referrals are tracked after the visit.

Stays with your clinicians

The visit itself, screening decisions and the prevention plan are delivered by your clinicians.

What your practice receives
  • Due-list built from coverage and visit history
  • Outreach and scheduling campaigns

FAQ

AWV questions.

What practices ask before launching AWV.

No. It is a prevention-planning visit. Patients should know that in advance to avoid confusion about what is covered.

Often. Visits commonly surface chronic conditions and needs that qualify for ongoing programs.

Your clinicians, or qualified clinical staff working under your supervision and protocols where contracted. Carevory runs the operations around them: eligibility, enrollment, outreach, scheduling, queues, documentation and reporting.

No. Payer rules, documentation and clinical decisions determine payment. We help you meet program requirements and give you clean records for billing review.
