Behavioral & socialPIN

A guide through serious illness — cancer, heart failure, dementia and more.

A serious diagnosis brings appointments, tests and decisions all at once. PIN pays for trained navigators to help patients and caregivers understand the path, coordinate across specialists and reach support services. We operate the navigation workflow alongside your clinical team.

Who qualifies

Patients and requirements.

Eligibility is confirmed against your own population before a pilot group is approached.

Check your population
  1. 01

A serious, high-risk condition expected to last at least three months

  2. 02

The condition puts the patient at significant risk of hospitalization, decline or death

  3. 03

An initiating visit by the billing practitioner

  4. 04

    Consent documented before services begin

What your patients experience

What changes for the person on the other end of the phone.

  • One person who helps them find their way
  • Appointments that happen in the right order
  • Caregivers who know what's next
  • Help reaching support services
How we run it

What the Carevory team does every month.

Your clinicians keep every clinical decision. We run the operations around them.

01

Start at diagnosis

Patients are referred to navigation after the initiating visit.

02

Map the journey

Upcoming appointments, tests and decisions are laid out with the patient.

03

Coordinate across teams

Navigators chase referrals, records and scheduling between specialists.

04

Support the family

Caregivers are included and connected to support services when wanted.

05

Document

Time and activities are logged for practitioner review.

Stays with your clinicians

Diagnosis, treatment planning and goals-of-care conversations remain with clinicians.

FAQ

PIN questions.

What practices ask before launching PIN.

They overlap but differ: navigation focuses on guiding the patient through a complex illness and its services, while care management centres on the care plan.

Trained auxiliary personnel, including community health workers and patient navigators, working under the billing practitioner's supervision.

Your clinicians, or qualified clinical staff working under your supervision and protocols where contracted. Carevory runs the operations around them: eligibility, enrollment, outreach, scheduling, queues, documentation and reporting.

No. Payer rules, documentation and clinical decisions determine payment. We help you meet program requirements and give you clean records for billing review.

Start with one workflow
