Behavioral & socialCHI

When housing, food or transport stand in the way of care.

Social needs block treatment plans every day. CHI pays for trained community health workers to help patients address the barriers that keep them from care — and to connect them with local resources. We help run needs assessment follow-up, navigation tasks and closed-loop documentation.

Who qualifies

Patients and requirements.

Eligibility is confirmed against your own population before a pilot group is approached.

Check your population
  1. 01

An initiating visit in which the practitioner identifies unmet social needs

  2. 02

Those needs significantly limit the ability to diagnose or treat the problem

  3. 03

Services performed by certified or trained auxiliary personnel, such as community health workers

  4. 04

    Consent documented before services begin

What your patients experience

What changes for the person on the other end of the phone.

  • Someone who helps with rides, food or benefits
  • Connections to local programs that fit
  • Follow-up until the need is met

How we run it

What the Carevory team does every month.

Your clinicians keep every clinical decision. We run the operations around them.

01

Pick up the need

Needs identified at the initiating visit are turned into navigation tasks.

02

Plan with the patient

Goals are set around the barrier that matters most to the patient.

03

Connect to resources

Referrals to food, housing, transport and benefit programs are made and tracked.

04

Close the loop

We confirm whether the resource actually helped and update the plan.

05

Document

Time and activities are logged for practitioner review.

Stays with your clinicians

The initiating visit, clinical goals and supervision remain with the billing practitioner.

FAQ

CHI questions.

What practices ask before launching CHI.

CHW staffing, training and local presence are scoped per engagement. Many practices pair their own local CHWs with our remote navigation support.

We maintain a directory for your service area and verify programs before referring patients.

Your clinicians, or qualified clinical staff working under your supervision and protocols where contracted. Carevory runs the operations around them: eligibility, enrollment, outreach, scheduling, queues, documentation and reporting.

No. Payer rules, documentation and clinical decisions determine payment. We help you meet program requirements and give you clean records for billing review.

Start with one workflow

Plan your CHI program with us.

Tell us about your patient population, systems and staffing. We'll scope a pilot group, the workflow and the reporting you'd see each month.
