Remote monitoringRPM

Readings from home, reviewed by people who know the patient.

RPM connects blood-pressure cuffs, glucometers, scales and pulse oximeters to your care team. We handle device logistics, patient onboarding, adherence outreach and the reading queue, so out-of-range values reach a clinician and routine readings never pile up.

Who qualifies

Patients and requirements.

Eligibility is confirmed against your own population before a pilot group is approached.

Check your population
  1. 01

An established patient with an acute or chronic condition

  2. 02

An order from the treating practitioner

  3. 03

A connected medical device that transmits physiologic data automatically

  4. 04

    Consent documented before monitoring starts

What your patients experience

What changes for the person on the other end of the phone.

  • A device that is set up for them
  • Someone who calls when readings stop
  • Fast follow-up when numbers are off
  • Fewer trips just to check vitals
How we run it

What the Carevory team does every month.

Your clinicians keep every clinical decision. We run the operations around them.

01

Order and ship

We coordinate device selection, shipping and returns with your chosen vendor.

02

Onboard at home

Patients are taught to take and send readings, often on a video or phone call.

03

Watch adherence

Patients who stop transmitting get a call before the month's day count is missed.

04

Route exceptions

Readings outside your thresholds are routed to clinicians under agreed protocols.

05

Close the month

Day counts, interactive communication and time are summarized for billing review.

Stays with your clinicians

Thresholds, clinical review of readings and treatment decisions remain with your clinicians.

What your practice receives
  • Device inventory and shipment tracking
  • A triaged reading queue
  • Adherence reporting by day count
  • Time logs for treatment-management codes

FAQ

RPM questions.

What practices ask before launching RPM.

We coordinate with the device vendor you choose or help you select one. Device costs, connectivity and logistics are itemized in the proposal.

Coverage hours and escalation protocols are agreed per engagement. RPM is not an emergency service, and patients are told what to do in an emergency.

Your clinicians, or qualified clinical staff working under your supervision and protocols where contracted. Carevory runs the operations around them: eligibility, enrollment, outreach, scheduling, queues, documentation and reporting.

No. Payer rules, documentation and clinical decisions determine payment. We help you meet program requirements and give you clean records for billing review.

Start with one workflow

Plan your RPM program with us.

Tell us about your patient population, systems and staffing. We'll scope a pilot group, the workflow and the reporting you'd see each month.
